The second wave of the pandemic is gaining momentum. The coronavirus is not giving up. What to do when the statistics are soaring and there are more and more people in our immediate environment who have had contact with it? It is time to realize that each of us can get infected and get sick. The probability is high, so we need to know what to do when we find out that we are "in the range" of the virus. More dangerous than COVID-19 may be a lack of reliable knowledge. That is why we have collected basic concepts and information on how to proceed in the event of infection.

Statistical data indicate that we should not panic. Analyses of the spread of coronavirus and the increase in the number of cases in the first wave of the pandemic give scientists grounds to claim that from 50 to 80 percent infected people experience COVID-19 asymptomatically or with minor symptoms of infection. In approximately 20 percent infected, the symptoms are more serious, and in several percent the disease takes on a severe and very severe form. However, these are only statistical data.

Scientific research on the health effects of virus infection SARS-CoV-2 are still ongoing, so we must avoid this risk at all costs. It is also worth organizing knowledge about how to proceed in specific cases.

DECLARATIONS OF WILLINGNESS TO RECEIVE THE VACCINE FOR PERSONS FROM 18 TO 69 YEARS OLD
People who have from 18 to 69 years old submit declarations of readiness to receive the vaccine on the website https://szczepimysie.pacjent.gov.pl/
Completing the form means that when vaccinations are extended to other age groups, people who have completed the declaration will be issued e-referrals for vaccination. Such e-referrals are the basis for registering at a vaccination point.

KIEDY ROZPOCZNĄ SIĘ SZCZEPIENIA?
Pierwsze dawki preparatu mają trafić do punktów szczepień January 25th. W grupie „1” po seniorach zaszczepieni zostaną nauczyciele i służby mundurowe. Szczepienie jest dobrowolne i bezpłatne. Pełna refundacja dotyczy wszystkich Pacjentów. Podanie preparatu zawsze poprzedzone jest kwalifikacją lekarską.

You must show up at the appropriate vaccination point, on the designated day and time. You must have an identity document with you, preferably an ID card. On site, you fill out a questionnaire, which is the first stage of qualifying for vaccination.

Pacjenci z wypełnioną ankietą przechodzą kwalifikację lekarską. Jeśli jej wynik jest pozytywny otrzymują szczepionkę podaną w ramię, w formie iniekcji domięśniowej. Do szczepienie przeciw COVID-19 nie trzeba być na czczo. Szczepionka może być podawana o każdej porze dnia.

Ze względów bezpieczeństwa, osoby którym podano preparat, powinny przez 15 minut pozostawać pod obserwacją. Pojawienie się nagłych, niepożądanych objawów należy natychmiast zgłaszać personelowi przychodni. Po przyjęciu szczepionki nie ma specjalnych ograniczeń dotyczących jedzenia i picia, czy wysiłku fizycznego. Przed opuszczeniem punktu szczepień warto potwierdzić termin podania drugiej dawki preparatu.

The basis for confirming the infection is MOLECULAR TEST RT-PCR towards SARS-COV-2. This is a so-called genetic test, the result of which we wait from 1 to 3 days. A referral for a free test can be issued by a doctor Primary Health Care /POZ/ after assessing the patient's health condition during a teleconsultation or a personal visit. People who experience symptoms characteristic of COVID-19, above all:  fever, cough, shortness of breath and breathing difficulties, muscle pain, loss of smell or taste. 

Referral to MOLECULAR TEST RT-PCR is displayed in the application www.gabinet.gov.pl. After receiving information about the referral, the patient should arrange a date for the examination at the selected collection point by phone and then go to the examination using their own transport, avoiding contact with other people.

In recent days, due to the large number of referrals for tests, there were long queues in front of the smear points and patients had problems reaching them by phone.

The patient can report to a paid clinic on his own. ANTIGEN TEST by registering on the website www.testprzesiewowy.pl. The SensDx technology test provides information within 15 minutes whether there is any active virus protein present in the swab. SARS-CoV2, which indicates the patient's infectiousness. The test is best performed 5-6 days after contact with an infected person or 2-3 days after the onset of infection symptoms. The test is of a screening nature, and its result is not a basis for intervention by sanitary and epidemiological services.

Diagnostic laboratories have the following available: SEROLOGICAL TESTS (IgG, IgM)which detect antibodies antiSARS-CoV2This allows us to determine whether a person has had an infection. COVID-19. Venous blood tests performed using the immunoenzymatic method (ELISA) or chemiluminescence (CLIA) are currently considered the only reliable tests for anti-SARS-CoV-2 antibodies. Experts from the World Health Organization (WHO) point out that they cannot be considered a method for detecting active cases of the disease. Antibodies appear quite late, and their highest level occurs about 2.5-3 weeks after infection.

About the test result RT-PCR the primary care physician learns from the application www.gabinet.gov.pl and informs the patient by phone. The patient can read such information on his/her Individual Patient Account.

In the case of a person with a positive coronavirus test result the doctor makes a decision about HOME INSULATION.

The primary care physician, assessing the patient's health condition, decides on the duration of home isolation, its possible extension or termination.

  1. If the patient does not experience symptoms typical of COVID-19 The default duration of home isolation is 10 days from the date of the test that led to the diagnosis. COVID-19At the end of this time, no earlier than on the 8th day the doctor provides the patient with a teleconsultation and informs him about the end of isolation. This fact is recorded in the medical documentation, but does not issue a medical certificate!
  • In a patient who had symptoms of a respiratory infection, the duration of isolation is extended. The doctor records such a decision in the medical documentation, issues a sick leave certificate for the period of illness and extends the isolation in the system. www.gabinet.gov.pl
  1. Home isolation lasts at least 13 days from the day the symptoms appeared (not from the date of the test!), if symptoms last longer than 10 days, the duration of isolation is extended according to the duration of the symptoms.

For example, if symptoms occur on day 5 of isolation and last for 12 days, the duration of isolation is 15 days from the date of symptom onset (not from the date of the test). If symptoms last for 20 days, the duration of isolation is 23 days from the date of symptom onset (not from the date of the test). 

Attention! The last three days of isolation must be asymptomatic – without fever, cough or shortness of breath.

ALL HOUSEHOLD MEMBERS ARE SUBJECT TO QUARANTINE FOR THE ENTIRE PERIOD OF HOME ISOLATION OF THE COVID-19 PERSON AND FOR 7 DAYS FROM THE DAY THE INFECTED PERSON WAS DECLARED HEALTHY. THESE PERSONS:

  1. They cannot leave the house or go shopping.
  2. They move around in rooms where the patient is not isolated or in their own home garden without contact with the patient or neighbours.
  3. You should ask for help by phone/email (family, friends, or MOPS), the person delivering the necessary products should leave them at the door of your house/apartment and notify in an established manner so as not to contact you directly.
THE FOLLOWING RECOMMENDATIONS APPLY DURING HOME ISOLATION:

  1. Prowadź dzienniczek samoobserwacji stanu zdrowia – wykonuj pomiar temperatury ciała, mierz saturację jeśli posiadasz pulsoksymetr. Bezbolesny i nieinwazyjny pomiar, który każdy może wykonać samodzielnie, pokazuje stopień wysycenia tlenem krwi tętniczej. Norma saturacji u osoby zdrowej oscyluje w granicach 95 – 99% (u seniorów 93 – 95%). Jeżeli saturacja jest niższa niż 90% mamy do czynienia z niedotlenieniem organizmu i wymaga to pilnej interwencji medycznej.
    If you suffer from hypertension or diabetes, monitor your BP and glycemia levels.
  2. Drink at least 1.5 liters of fluids a day, eat 3-4 easily digestible meals.
  3. Stay (24/7) in one room where no other household members are present.
  4. Other household members should NOT come closer than 2 metres from you.
  5. You should NOT have contact with pets – they can transmit the virus to healthy people on their fur that you touch.
  6. If another person enters the room you are in, they should wear a mask over their mouth and nose.
  7. DO NOT take things out of the room you are using.
  8. DO NOT enter other rooms, especially if someone is there.
  9. If there is a shared toilet/bathroom in the apartment, use it separately, preferably as the last household member, wash the devices you use after yourself, disinfect the toilet with household products, wash your hands thoroughly before leaving. Use only your own towels, hang them in a different place than the towels of other household members. Keep toiletries, e.g. a toothbrush, in the room where you are isolating.
  10. Open the window often, air the room. If you have to leave the room, e.g. to the bathroom, put on a mask over your mouth and nose.
  11. DO NOT use fans, air conditioning, or air purifiers.
  12. Change bed linen and pajamas every 2 days, and towels daily.
  13. Place used bedding, towels, and dirty clothes in a plastic bag in the room, and when it is full, hang it outside the door so someone else can do the laundry.
  14. Throw your trash in a plastic bag (ask for an empty bag to be hung on the doorknob) and when it is full, hang it outside the door so someone else can throw it in the trash.
  15. Use separate cutlery, plates, cups, glasses.
  16. Consume prepared meals, water, and medications in isolation, and ask for them to be delivered to your door.
  17. Put dirty dishes outside the room door so that someone else can wash them in a dishwasher at a minimum temperature of 60⁰C using detergent or by hand using dishwashing liquid and hot water. After washing dishes, the person washing them must wash their hands with soap and water.

 

HOW TO CLEAN IN AN APARTMENT WHERE ONE (TWO SICK) PERSON IS ISOLATED AND THE REST ARE HEALTHY?

  • Kilka razy dziennie dezynfekuj poprzez przecieranie chusteczkami nasączonymi środkiem dezynfekcyjnym: – klamki do drzwi, blaty stołów, uchwyt lodówki, szafek, rączka czajnika, armatura łazienkowa, toalety i uchwyty toaletowe, telefony, klawiatury, piloty telewizyjne, tablety, stoliki nocne, włączniki światła – dezynfekcję wykonuj w gumowych rękawiczkach.
    DO NOT wipe excess disinfectant from surfaces.
  • Pranie ubrań, pościeli: – pierz rzeczy w oddzielnym wsadzie, bez ubrań innych domowników, – wysyp ubrania do pralki bezpośrednio z worka, bez ich dotykania, – ubrania i pościel pierz w temperaturze najwyższej dopuszczalnej dla pranych tkanin, ale nie niższej niż 60 stopni C, z użyciem detergentu, przez co najmniej 40 min. – używaj ubrań, które można prać w temperaturze 60⁰C – po załadunku pralki umyj lub zdezynfekuj ręce.
    DO NOT send items to dry cleaners.
  • Wyrzucanie śmieci – osoba wyrzucająca śmieci z twojego domu/ mieszkania odbiera worek spod drzwi pokoju w gumowych rękawiczkach, pakuje je w drugi worek, wyrzuca do śmietnika. Po zdjęciu rękawic wyrzuca je, myje lub dezynfekuje ręce.

Home isolation imposes special responsibility on the people subjected to it. Patients with a positive test result SARS-CoV-2 must make every effort to limit the risk of infecting other people in their environment. Due to the possibility of endangering the health and lives of other people, disregarding sanitary regulations is subject to criminal and administrative liability. According to Article 165 of the Penal Code, the crime of "bringing danger to the life or health of many people by causing an epidemiological threat or the spread of an infectious disease" faces up to 8 years of imprisonment. People who violate the regulations on home isolation or quarantine may also be punished by the sanitary inspector with an administrative penalty of up to PLN 30,000.

Sanepid is always notified about issuing a referral for a molecular test. Inspectors monitor whether the patient has reported for the test, and if the result is positive, they begin an epidemiological investigation to impose quarantine on contacts.
