Thanks to individually selected tests and conversations with medical staff, patients have the opportunity to better understand their health and learn how to improve it. People aged 20 to 49 can use the new program once every 5 years. After the age of 50, every 3 years. Patients who participated in Prevention 40+ must wait a year after completing the tests.&nbsp;<\/p>\n\n\n\n<p>The implementation of the program would not be possible without the involvement of the entire team <strong>NZOZ Your Doctor<\/strong>. A special role is played by nursing staff,<strong> <\/strong>who performs measurements and some tests and conducts summary visits and educational advice. It is the nurses in the new program who actively support patients in taking care of their health.&nbsp;<\/p>\n\n\n\n<p>An important element of the campaign is health education, which helps build lasting habits and prevent the development of lifestyle diseases. Educational activities using a podcast for patients fit perfectly into such program assumptions. <a href=\"https:\/\/twojlekarz.net\/en\/medyczny-podcast\/\"><strong>WHAT DOES YOUR DOCTOR SAY?<\/strong><\/a>. Broadcasts with specialists created by <a href=\"http:\/\/www.promocjazdrowia.org.pl\"><strong>Health Promotion Foundation<\/strong><\/a> promote reliable medical knowledge and health-promoting behaviors.&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>HOW TO JOIN THE PROGRAM?<\/strong><\/h2>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>1. Health Survey&nbsp;<\/strong><\/h3>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Log in to your IKP Patient Online Account (www.pacjent.gov.pl) or open the Moje IKP application.<\/li>\n\n\n\n<li>Go to the tab <strong>Prevention<\/strong><\/li>\n\n\n\n<li>Complete the Adult Health Assessment Questionnaire.<\/li>\n\n\n\n<li>Please wait for us to contact you (up to 30 days)&nbsp;<\/li>\n<\/ul>\n\n\n\n<p>If you do not have access to the Online Patient Account or need help, our medical staff is here to support you. Simply report to the treatment room at the NZOZ Tw\u00f3j Lekarz clinic.&nbsp;<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>2. One month to decide on next steps<\/strong><\/h3>\n\n\n\n<p>After the patient submits the survey, the clinic has 30 days to plan further actions. This is the time to analyze the answers, plan contact with the patient, issue referrals and set possible dates for tests and visits.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>3. Scope of research tailored to your needs<\/strong><\/h3>\n\n\n\n<p>The program provides for a set of diagnostic tests, which may be extended depending on age, gender and responses to the survey.<\/p>\n\n\n\n<p>Basic research includes:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>blood count,<\/li>\n\n\n\n<li>glucose level,<\/li>\n\n\n\n<li>creatinine with eGFR,<\/li>\n\n\n\n<li>lipid profile,<\/li>\n\n\n\n<li>TSH,<\/li>\n\n\n\n<li>general urine examination.<\/li>\n<\/ul>\n\n\n\n<p>Extended tests may include:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>ALAT,&nbsp;<\/li>\n\n\n\n<li>ASPAT,&nbsp;<\/li>\n\n\n\n<li>GGTP,<\/li>\n\n\n\n<li>PSA designation (for men),<\/li>\n\n\n\n<li>testing for HCV infection,<\/li>\n\n\n\n<li>lipoprotein (a) assay,<\/li>\n\n\n\n<li>occult blood in the stool,<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>4. Summary visit and Individual Health Plan<\/strong><\/h3>\n\n\n\n<p>Before the summary visit, each participant of the program will undergo measurements in the treatment room. Then they will receive referrals for laboratory tests. After the tests are completed, we will conduct a summary consultation, during which the medical staff will discuss the results and develop <strong>Individual Health Plan.<\/strong> It is a personalized set of recommendations and actions to support your health every day.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>5. Health education \u2013 a key element of the program<\/strong><\/h3>\n\n\n\n<p>An integral part of the program are at least two educational consultations, conducted by medical personnel. They may concern:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>healthy diet,<\/li>\n\n\n\n<li>increasing physical activity,<\/li>\n\n\n\n<li>quitting smoking,<\/li>\n\n\n\n<li>limiting alcohol consumption,<\/li>\n\n\n\n<li>prevention of cardiovascular and metabolic diseases.<\/li>\n<\/ul>\n\n\n\n<p>Health education is an investment in the future.
