How to register a newborn for a clinic?

Parents or guardians who wish to enroll their child in our clinic should submit a declaration to the selected NZOZ "Twój Lekarz" (Your Doctor) facility (the form is available in the "Downloads" tab). When completing the declaration, it is necessary to have the details of the person who will register the child for insurance (name, surname, PESEL number). When registering a newborn, it is also necessary to provide an immunization card (also known as an immunization card).
