Each of the NZOZ "Twój Lekarz" facilities has access to the Electronic Verification of Beneficiary Entitlements (eWUŚ). Thanks to this, the staff can immediately check whether a given person is insured and entitled to free medical care - without the need for them to present certificates or RMUA forms.
A patient reporting to the clinic only provides his/her PESEL number at the registration desk and confirms his/her identity (using an identity card, passport or driving license, and in the case of a child subject to compulsory schooling up to 18 years of age - a valid school ID).
Yes, the patient has the right to choose the specialist to whom the referral will be made.
There is a call center in the facility - if the line is busy or the call is not answered, after 3 seconds the call is transferred to a free call center agent.
If it is necessary to justify an employee's temporary incapacity to work (caused by illness or the need to provide care to a sick family member), a certificate will be issued on the ZUS ZLA form.
In order for such a certificate, i.e. sick leave, to be valid, it is necessary to provide the following data on the ZUS ZLA form:
by the person issuing the ZLA:
- name and surname of the insured person,
- the address where the person will stay during the period of sick leave,
- date of discharge, medical indications and discharge letter code,
by patient:
- PESEL number of the insured person (or passport series and number if no PESEL number has been assigned),
- Tax Identification Number of the employer.
If you are employed by more than one company, it will be necessary to issue a separate certificate for each employer, i.e. provide the Tax Identification Number (NIP) of each contribution payer.
The ZUS ZLA form is intended for employees in an employment relationship. People employed in another form do not receive such a form. Those insured in KRUS may - on the basis of a medical certificate of temporary incapacity for work due to illness, issued on the ZUS ZLA form - apply for sickness benefit. It is due for each day of incapacity for work lasting continuously for at least 30 days, but no longer than 180 days.
No, before the test you must absolutely take the medication prescribed by your doctor and drink some water. Fasting means that you cannot eat a meal, drink coffee or tea before the test, which could simply falsify the test result. In the case of people with chronic diseases, e.g. diabetes, if possible, you should postpone your first meal and insulin intake until after the test or take this into account when reading the results.
A referral to a specialist clinic is valid from the time of issue until the cure of a given disease entity. Therefore, in the new calendar year there is no need to prescribe a referral.
- Gynecological ultrasound Before a vaginal examination, the bladder should be emptied. If the examination is performed through the abdominal wall, a full bladder should be presented.
- Abdominal ultrasound The day before the examination, you should have your last meal at around 6:00 PM (if the examination is in the morning), then you can drink still water and tea. To exclude flatulence, it is also recommended to take 2 Espumisan capsules 3 times a day on the day before the examination. Abdominal ultrasound is performed on an empty stomach. During the examination of the small pelvic organs, it is absolutely necessary to have a full urinary bladder. Infants should be examined before feeding time.Preparing children for abdominal ultrasound examination. You should bring previous imaging test results and other medical documentation regarding diagnosis and treatment to the examination.Newborns and infants: – No need to go on a diet before the test; – Please bring something to drink (tea, water, or milk); – A blanket or a board, a favorite toy; – A pacifier (if your child uses a pacifier) Preschool children: – For 2 days before the examination, do not give the child raw fruit, vegetables, carbonated fluids, or flatulence-inducing foods; follow a light diet; Two days before the examination and in the morning on the day of the examination we administer Espumisan at a dose of 100mg/1ml: Espumisan 3×25 drops; – The child should not eat for about 4 hours before the test; – The child should be given 250 ml of water (water, tea – NOT JUICE) 2 hours before the test; do not give fluids immediately before the test; – The test is performed with a full bladder (the child should report the need to urinate); – The day before, and preferably in the morning on the day of the test, the child should have a bowel movement. School-age children: – For 3 days before the examination, do not give your child raw fruit, vegetables, carbonated fluids, flatulence-inducing foods, or chewing gum; follow a light diet; Two days before the examination and in the morning on the day of the examination, Espumisan is administered at a dose of 100 mg/1 ml: Espumisan 3×25 drops; In people over 14 years of age, suffering from flatulence or obese: Ulgix Max Wzdcia 2×1 caps. and in the morning on the day of the examination 1 caps. – The test should be performed approximately 6 hours after a meal (on an empty stomach) – The child should be given 250-500 ml of water (water, tea – NOT JUICE) 2 hours before the test; do not give fluids immediately before the test; – The test is performed with a full bladder (the child should report the need to urinate); – The day before, and preferably in the morning on the day of the test, the child should have a bowel movement.
- Breast ultrasound The test does not require any prior preparation. It is best to do it in the first phase of the menstrual cycle. Immediately before the test, do not use creams and balms, as well as deodorants and talcs (especially under the arms).
- Heart ultrasound The examination does not require prior preparation.
- Joint ultrasound The examination does not require prior preparation.
- Ultrasound of salivary glands The examination does not require prior preparation.
- Thyroid ultrasound The examination does not require prior preparation.
- Urological ultrasound To eliminate gas from the abdominal cavity, you should refrain from eating for at least 8 hours or take 6-8 tablets of medicinal charcoal. The test is performed on an empty stomach. Before the test, you should also not drink carbonated drinks or smoke cigarettes.
- Ultrasound of lymph nodes The examination does not require prior preparation.
- Ultrasound of hips in children The examination does not require prior preparation.
Parents or guardians who wish to provide care to their child at our clinic should submit a declaration to the selected NZOZ "Twój Lekarz" unit (the form is available in the "downloads" tab). When filling out the declaration, it is necessary to have the data of the person who will register the child for insurance (name, surname, PESEL). When registering a newborn, it is also necessary to provide an immunization card (so-called vaccination card) issued by the hospital where the child was born. After completing the formalities, your child will be provided with care:
- community and family midwife – patronage (from 4 to 6 visits until the age of 2 months);
- healthy children clinic – medical care, vaccinations, check-ups;
- sick children's clinic.
In-person visits are available Monday through Friday from 8:00 a.m. to 6:00 p.m.
