ELECTRONIC METHOD USING A TRUSTED SIGNATURE (ePUAP)
The most modern method is to submit the declaration electronically using a trusted signature. Simply download the form from the clinic's website, complete it on your computer, and then sign it electronically. Send the signed document to the following email address: deklaracje@twojlekarz.net. This is the fastest and most convenient method, allowing you to avoid printing and scanning documents.
ELECTRONIC METHOD WITH A STANDARD SIGNATURE
If you do not have a trusted signature, you can use the electronic method with a regular signature. Download the form, fill it out on your computer, then print it out and sign it by hand. Send a scan of the signed form to the same email address. This is also a quick way to submit a declaration without having to visit a clinic.
TRADITIONAL METHOD – PERSONAL DOCUMENT SUBMITTAL
For those who prefer personal contact, NZOZ Twój Lekarz offers the possibility of submitting a declaration directly at the facility. Download the form from the website, fill it out, print it out and sign it, then bring it to the nearest clinic. The registration team will be happy to help with any questions or concerns.
