Based on the questionnaire responses, appropriate diagnostic tests are ordered. Their execution is crucial to the entire process. Without a complete set of up-to-date results, it is impossible to reliably assess disease risk or plan further preventative measures. Missing any of these tests leaves the health picture incomplete. At NZOZ Your Doctor facilities, the program is implemented as part of the family doctor's daily care for patients enrolled in the network. Patients proceed through a structured process. First, a questionnaire is reviewed, followed by referrals for tests and an invitation to a treatment room for a basic health assessment. After completing the laboratory tests, a consultation with the doctor takes place, during which the results are discussed and further action is determined.
Continuity of care is crucial. The physician reviewing laboratory test results has access to treatment history and previous results, allowing them to more accurately assess changes and propose appropriate actions.
