POZ – Primary Health Care is more than just a chance to get free medical advice. A comprehensive service reimbursed by National Health Fund insured persons, also includes nursing care, and in the case of women, the possibility of using the services of a midwife. It is worth remembering this when we intend to submit a declaration to a specific facility.

Functioning POS is based on the right to choose: doctor, nurse and midwife of Primary Health Care. We make this choice by filling out a declaration to the clinic. How to do it?

Due to the COVID-19 pandemic, we recommend:

– accurate completion, printing and signing of the form,

– sending a scan of the signed declaration to the following address: declarations@localhost

ATTENTION!

IN THE DOCTOR, NURSE AND MIDWIFE SELECTION FIELDS, THE NAMES OF THE MEMBERS OF THE NZOZ YOUR DOCTOR TEAM RESPONSIBLE FOR SUPERVISING THE INDIVIDUAL DEPARTMENTS WILL APPEAR. ENTERING A SPECIFIC NAME DOES NOT LIMIT PATIENTS FROM USING THE SERVICES OF THE ENTIRE FACILITY STAFF.

People who have Individual Patient Accounts can submit declarations to the clinic electronically, via this system. However, it is important to include the names from the list available in the active declaration form in the nurse and midwife selection fields. NEW NZOZ YOUR DOCTOR CLINIC COMING SOON IN MIRKÓW

The new NZOZ "Twój Lekarz" (Your Doctor) clinic is scheduled to open in Mirków, in the Długołęka commune, in the second half of November. The new facility is conveniently located near the Mirków train station.
