COORDINATED CARE IN YOUR DOCTOR’S CLINICS!

The NZOZ Your Doctor network is the first in Wrocław and its immediate vicinity to implement the Coordinated Care Program in Primary Health Care. This is a tool that allows for comprehensive patient care and strengthening of prevention in Primary Health Care, as well as a quick response in the event of detecting a serious disease.

Thanks to the implementation of coordinated care, patients with some chronic diseases
will gain access to more tests and comprehensive and continuous care provided
by primary care staff: a doctor and a nurse, and – in some cases – by a dietitian
or medical specialists.

For each patient covered by the coordinated care program, a medical care plan will be created, which will allow for continuous and predictable systematization of the patient's visits to the clinic, as well as monitoring the patient's test results and the entire treatment process. The coordinator will support the patient in implementing the plan developed by the family doctor. He will also provide all necessary information and plan specific dates for educational visits, specialist or dietetic consultations.  

In NZOZ facilities, your doctor will provide coordinated care in the following areas:

– diagnosis and treatment hypertension, heart failure, chronic ischemic heart disease and atrial fibrillation,

– diagnosis and treatment diabetes,

– diagnosis and treatment bronchial asthma and chronic obstructive pulmonary disease,

– diagnosis and treatment hypothyroidism and diagnosis of single and multiple thyroid nodules.

The coordinated care program is not intended to apply to patients under the constant care of specialist clinics. Their inclusion in the program may be decided by a primary care physician. 

Coordinated care in clinics Your Doctor in addition to primary care doctors and nurses, they will implement: endocrinologist, diabetologist, cardiologist and pulmonologist and Dietitians. Specialist equipment has been purchased to implement the project, allowing doctors to expand diagnostics in individual diseases. The project will take many months to implement, but from early November 2022, patients will be gradually invited to participate in expanded, comprehensive primary care.

Its goal is to create an individualized health plan and provide real support to the patient in changing their lifestyle.

A visit to a new doctor often begins with a complete health history: where diagnostic tests were performed, what the results were, and what medications were prescribed. Some documents are kept at home, others at the previous facility.
