A LESSON FOR THE HEART!

Target group:

Students of primary schools from the Wrocław District (divided into two age groups: grades I-III and grades IV-VIII),

Parents of students, Teachers.

Main objectives of the program:

Health education for children and adolescents regarding the prevention of cardiovascular diseases, hypertension and hypercholesterolemia.

Developing positive eating habits and promoting a healthy lifestyle among children and adolescents.

Strengthening health-related knowledge and attitudes among children in grades I-VIII.

Method of implementation:

Information campaign: Using a variety of communication channels (local media, websites, posters, leaflets) to disseminate information about the programme.

Educational activities: Implementation in the form of lectures, workshops, talks, educational games and activation activities in the school environment, adapted to the age of the participants.

  • For grades I-III: Classes conducted in the form of play, conversation, taking into account education on positive eating habits and other elements of health prevention.
  • For grades IV-VIII: Strengthening knowledge about a healthy lifestyle through practical workshops, discussions and games, and possibly also lectures.

Educational materials: Creation and distribution of various educational materials such as posters, leaflets, colouring books, puzzles for younger students and scripts and electronic materials for older students.

Podcasts: Preparation and provision of audio materials related to health topics discussed during classes.

Checking and confirming students' knowledge: Implementation of pre- and post-knowledge tests for students in grades IV-VIII.

Online activities: Providing educational content also remotely, using available online tools and platforms.

Program scope: Around 20 schools from the Wrocław District will be included in the programme, with an expected number of students of around 16,000. It is planned to hold two educational meetings in each school, separately for younger and older groups of students.

"Health policy program for the prevention of cardiovascular diseases, hypertension and hypercholesterolemia for the residents of Wrocław County for 2023–2025" is a comprehensive educational undertaking aimed at promoting knowledge about a healthy lifestyle and prevention of cardiovascular diseases, hypertension and hypercholesterolemia among children and adolescents of primary schools. It uses various methods and educational forms, adapted to the age of participants, and a broad information campaign. It includes stationary and online activities, providing access to educational content for the widest possible audience.

Lyme disease is the most common tick-borne disease in Europe and North America. In Poland, the number of cases is increasing year by year. Although Lyme disease is easy to treat in the early stages, a delay in diagnosis can lead to serious complications, such as neurological problems, joint pain, or heart problems.

The health policy program for the prevention of tick-borne diseases (Lyme disease) for the years 2023–2025 for the residents of Wrocław County is a response to the growing threat of Lyme disease.

1. Purpose:

Improving knowledge about Lyme disease among the inhabitants of the Wrocław district, mainly among people exposed to contact with ticks, such as forest workers, farmers and hunters.

2. Recruitment and qualification:

Qualification of participants from the high-risk group from the Wrocław district, including those who were bitten by a tick but did not undergo testing. Medical qualification is carried out by the staff of the NZOZ Twój Lekarz network implementing the program.

3. Medical services provided to patients qualified for the program:

  • Diagnostic and therapeutic consultation:

Consultations are held at NZOZ Twój Lekarz facilities, Monday through Friday, 8:00 a.m. - 6:00 p.m., in the form of a personal visit or teleconsultation. The purpose of the consultation is a detailed medical interview and, in the event of suspected Lyme disease, ordering tests.

  • Diagnostic tests for Lyme disease - ELISA test and Western blot test
  • Specialist consultation:

Based on the test results, all Program participants whose condition requires it are referred to a physician specializing in infectious diseases.

4. Information and educational activities:

All participants of the Program receive information materials on the prevention of tick-borne diseases and how to proceed in the event of a tick bite. An integral part of the information campaign is a series of podcasts CO NA TO TWÓJ ELEKARZ devoted to counteracting tick-borne diseases.

The program is financed from the budget of the Wrocław District.

NZOZ Your Doctor is a grantee in the project entitled:Implementation of e-services in primary healthcare facilities and their integration with the e-health system” (“POZ e-services”), financed from European Funds.

Our facility received a grant to expand its office system and purchase IT equipment that enables the creation and sharing of electronic medical records (EMD) and the provision of e-services. 

The project is financed by the European Union as part of the REACT-EU package supporting counteracting the effects of the COVID-19 pandemic. 

The project is implemented by the Ministry of Health and its value is PLN 99,589,028.57. 

The aim of the project is to support the digitalization of primary healthcare facilities in the provision of e-services, integration with the e-health system and improvement of the provision of services.

How should I report irregularities?

The Infrastructure and Environment Programme Managing Authority attaches particular importance to ensuring that projects under the Programme are implemented in an ethical, open and transparent manner. Therefore, a simple tool has been created to report any irregularities concerning projects financed from funds from the Infrastructure and Environment Programme.

The signals about irregularities transmitted will allow for the introduction of appropriate remedial measures and will contribute to ensuring the highest standards of project implementation.
