GDPR Information
GDPR - INFORMATION CLAUSE FOR PATIENTS
In accordance with Article 13 of the General Data Protection Regulation of 27 April 2016 (OJ EU L 119 of 04.05.2016), I hereby inform that:
1) The administrator of your personal data is:

3) Your personal data will be processed for the purpose of conducting medical activities by the Administrator., in particular for the purpose of providing medical services by the Administrator and maintaining medical documentation required by law.
4) The legal basis for processing your data in the scope of conducting medical activities, including in the scope of maintaining medical records, is art. 6 sec. 1 letter c of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC - hereinafter referred to as: GDPR, in connection with the provisions of the Act of 15 April 2011 on medical activity and the Act of 6 November 2008 on patients' rights and the Patients' Ombudsman. The basis for data processing to the extent necessary to protect the vital interests of the patient may also be art. 6 sec. 1 letter d of the GDPR. To the extent that the processed data include special category data, the legal basis for data processing is art. 9 sec. 2 letters c and h of the GDPR.
5) The recipients of your personal data will only be entities authorized to obtain personal data under the provisions of law, including the National Health Fund, analytical laboratories and other entities whose participation is necessary for the implementation of medical services.
6) Your personal data will generally be stored for the period for which medical documentation is stored, i.e. for a period of 20 years from the end of the calendar year in which the last entry was made, in any case for the period specified in Article 29 of the Act on Patients’ Rights and the Patients’ Ombudsman.
7) You have the right to request from the administrator access to your data, to rectify it, to request its deletion, the right to limit its processing and the right to transfer data.
8) You have the right to lodge a complaint with the supervisory authority.
9) Providing your data is voluntary, however, failure to provide it will result in the inability to provide the medical service.
10) In connection with the processing of personal data, you will not be subject to decisions made in an automated manner (without human intervention). Your personal data will also not be used for profiling.
11) We do not transfer your data outside Poland / EU / European Economic Area.

3) Personal data will be processed for the purposes of:
and. to conduct the current recruitment process (legal basis: Article 221 § 1 of the Labour Code in connection with Article 6 paragraph 1 letter b of the GDPR Regulation), i.e. in order to conclude a contract, while other data, including contact details, are provided on the basis of consent (Article 6 paragraph 1 letter a of the GDPR), which may be withdrawn at any time. Providing data is voluntary, but necessary to take part in the recruitment process;
b. conducting future recruitment procedures (legal basis: Article 221 § 1 of the Labor Code in connection with Article 6 paragraph 1 letter a of the GDPR Regulation) if voluntary consent is given which may
be revoked at any time. Providing data is voluntary, but necessary to participate in future recruitments;
c. processing of image or health data in the event of expressing consent by voluntarily supplementing the Candidate's application documents (CV) with special category data (legal basis: art. 222 § 1 of the Labor Code in connection with art. 6 sec. 1 letter a of the GDPR Regulation). Consent to the processing of special category data may be withdrawn at any time;
d. any determination, pursuit or securing of claims (legal basis: art. 6 sec. 1 letter f) of the GDPR), i.e. the legitimate interest of the Controller.
4) Your personal data will be stored by:
and. specified in letter a) for the period of the recruitment process;
b. for the purpose specified in points b) and c) for a period of 2 years from the date of consent or until the consent is withdrawn;
c. for the purpose specified in letter d) for the period of limitation of claims arising from legal provisions.
5) Your personal data may be made available to: recipients of data authorized under separate
regulations (ZUS, US, etc.), entities cooperating with the Controller on the basis of data joint-administration agreements and entities which, on the basis of a processing entrustment agreement, perform activities supporting the Controller's activity at its request, including, among others, suppliers and service providers of IT systems and companies providing other services to the Controller necessary to perform agreements (e.g. accounting offices).
6) You have the right to request from the administrator access to your personal data, the right to rectify, delete or limit processing, the right to object to processing,
the right to transfer data, the right to withdraw consent at any time.
7) You have the right to lodge a complaint with the supervisory authority.
8) Providing personal data is obligatory under the law and in the remaining scope it is
voluntary.
9) In connection with the processing of personal data, you will not be subject to decisions made in an automated manner (without human intervention). Your personal data will also not be

GDPR - INFORMATION CLAUSE - MONITORING
In accordance with art. 13 sec. 1 and 2 of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of their personal data and on the free movement of such data, and repealing Directive 95/46/EC, OJ EU L 119 of 4 May 2016 (hereinafter referred to as GDPR), I inform you that:
1. The administrator of your personal data is NZOZ "Twój Lekarz sp. z o. o." with its registered office in Kobierzyce (55-040) at ul. Witosa 5.
2. We have appointed a Personal Data Protection Inspector. If you have any questions regarding the method and scope of processing your personal data in the form of video monitoring, as well as your rights, you can contact the Personal Data Protection Inspector - contact to the Personal Data Protection Inspector Katarzyna Idziak, tel. 790 494 084, e-mail: iod@twojlekarz.net or in writing to the address of the administrator's registered office.
3. The following areas are subject to video monitoring: waiting room, registration in all NZOZ "Twój Lekarz sp. z o. o." facilities.
4. All data recorded by video surveillance cameras are saved and available for a maximum period of 3 months. After this period, the data is automatically overwritten. Only the image (without sound) is registered and saved on the medium.
5. For evidentiary purposes, events recorded by the video monitoring system which, among other things, pose a threat to public safety and order, destruction and theft of property are secured at the request of the authorities conducting the proceedings.
6. Each securing of events recorded by video monitoring is carried out upon a written request submitted to the Head of the unit. Secured data from video monitoring are made available only to bodies conducting proceedings in the case of the recorded event, e.g. police, prosecutor's office, courts, which operate on the basis of separate regulations. In the event of an incident, the data are stored until the case is clarified or until the relevant proceedings are completed.
7. Your personal data will be processed in order to ensure public safety and order and to protect persons and property on the basis of Article 6 paragraph 1 letter e of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (GDPR) and on the basis of Article 23a of the Act on Medical Activity of 15 April 2011.
8. Your personal data may be made available only to authorities entitled to receive them under applicable law.
9. Your personal data will not be transferred to countries outside the European Economic Area, which consists of the European Union countries and Iceland, Norway and Liechtenstein.
10. No automated decisions will be made regarding you (decisions without significant human involvement), including your personal data will not be subject to profiling.
11. You have the right to lodge a complaint with the supervisory authority – the President of the Personal Data Protection Office, if the processing of your personal data violates the provisions on data protection.
GDPR - INFORMATION CLAUSE - DECLARATION OF VACCINATION REGISTRATION
In accordance with Article 13 of the General Data Protection Regulation of 27 April 2016 (OJ EU L 119 of 04.05.2016), I hereby inform that:

3) Your personal data are processed for the purpose of handling, organizing and implementing the process of vaccination against the disease caused by SARS-CoV2 virus infection (COVID-19).
4) The legal basis for the processing of your personal data is Article 6 paragraph 1 letter a of the GDPR in connection with the consent granted to the processing of personal data for the purpose specified in point 3.
5) The recipient of your personal data may be entities authorized to obtain personal data under the provisions of generally applicable law. The data may be transferred to entities with which the personal data controller has concluded an agreement for the processing of personal data.
6) Your personal data will be stored for the period necessary to implement the National Vaccination Program.
8) Your personal data will not be used for automated decision-making or profiling.
9) Your personal data will not be transferred to third countries or international organizations.
10) You have the right:
• access to your data;
• to rectify (correct) your data;
• processing restrictions;
• the right to object;
• the right to delete data;
• the right to withdraw consent at any time without affecting the lawfulness of processing based on consent before its withdrawal.
11) If you believe that the processing of your personal data violates the provisions of the law, you have the right to lodge a complaint with the supervisory authority, i.e. the President of the Personal Data Protection Office (address: ul. Stawki 2, 00-193 Warsaw).
12) Providing personal data is voluntary, but refusal to provide it will prevent us from contacting you for purposes related to the handling, organization and implementation of the vaccination process against the disease caused by SARS-CoV2 virus infection (COVID-19).
