Coordinated care in primary care. What does this mean for patients?

Our clinics implement the Coordinated Care Program under a contract with the National Health Fund (NFZ). This modern model of care helps patients access consultations, diagnostics, and treatment for chronic conditions more conveniently and without long wait times. At NZOZ Your Doctor clinics, patients remain under the care of a primary care physician who collaborates with specialists and coordinates treatment and preventative care.

As part of the program, patients can benefit from advice on:

  • cardiologist,
  • endocrinologist,
  • diabetologist,
  • nephrologist,
  • pulmonologist,
  • dietetics.

Care is provided in cooperation with a primary care physician and a team of specialists.
