Primary Health Care (PHC) in NZOZ "Your Doctor".
As part of the primary healthcare clinic, we provide health services to insured persons who have chosen a doctor, nurse and midwife through submitting an appropriate declaration to NZOZ "Your Doctor". By becoming a patient of NZOZ Your Doctor you join the group of people who are "well cared for" and you can use all the branches of our network.

Patients registered with NZOZ Twój Lekarz have the option of ordering prescriptions for medications recorded in their medical records that they take on a regular basis.

Our patients can consult primary care physicians – family medicine specialists, pediatricians, and internists in all facilities in our network, regardless of where the declaration was submitted.
The Primary Care Clinic is:
- medical consultations in the field of prevention, diagnosis and treatment of diseases,
- care of a community and family nurse and midwife,
- vaccinations and check-ups at the well-child clinic,
- childbirth school.
RULES FOR USING NIGHT AND HOLIDAY HEALTH CARE
Night and holiday health care are primary health care services provided from Monday to Friday from 6:00 p.m. to 8:00 a.m. the following day, and on Saturdays, Sundays and other public holidays from 8:00 a.m. to 8:00 a.m. the following day.
The assistance is provided at night care points and is not related to the primary health care physician with whom we are registered. Insured persons may use the assistance of any night care point in the event of a deterioration in their health. Information on where night and holiday medical and nursing care is provided in a given region can be obtained by the patient from their primary health care physician, in the appropriate provincial branch of the National Health Fund or its delegation.
Night and holiday medical care services are free (for insured persons) and provided without a referral.
The doctor on duty at the clinic provides advice:
- in an outpatient setting,
- at the patient's home (in medically justified cases),
- by phone.
On public holidays, after 6:00 p.m., patients are also entitled to nursing procedures as part of the advice provided by the doctor on duty at the primary care facility, as well as procedures resulting from the continuity of treatment. These procedures can be performed by a nurse in the treatment room or at the patient's home.
Emergency medical services – emergency number – 999 or 112 – whenever you feel that your life or someone else’s life is in danger, you should definitely call the emergency medical services number.
An ambulance equipped with specialist equipment has a professional rescue team that will quickly arrive at the specified address. The dispatcher who receives the call can assess whether the patient's health condition requires sending an ambulance or referral to overnight mobile or outpatient care.

Vaccinations for children
Note: Most vaccines are available at our clinic’s vaccination point, which ensures they are stored correctly and saves you time when purchasing them.
According to the WHO (World Health Organization), pneumococci cause the greatest number of deaths and disease complications in children aged 0 to 5 years. A child of any age can be vaccinated against pneumococci.
Age: 6 weeks – four doses. Vaccination can start after the child is 6 weeks old. Four doses are given: three doses every 6 weeks, the fourth one after a year.
Age: 6 months – three doses. Starting after the age of 6 months, three doses are given: two every 6 weeks and one after a year.
Age: 12 months – 2 doses. When starting vaccination after the first year of life, two doses are given every 6 weeks.
Age: over two years of age – 1 dose.
Vaccines: Prevenar, Synflorix.
Rotavirus vaccinations protect children from severe forms of diarrhea and usually help avoid hospitalization in the youngest children. These are oral vaccines. We have two types of vaccines: Rotarix (two-dose) every 6 weeks and Rotateq (three-dose), also every 6 weeks. This vaccine contains more antigens.
NOTE: Rotavirus vaccinations must be administered before the child is 6 months old.
Chickenpox and shingles are diseases that occur worldwide and are caused by the same virus, Varicella-zoster virus. Chickenpox usually occurs in children.
Routes of infection
The virus that causes chickenpox and shingles is spread by droplets, direct contact, and indirectly by contact with objects freshly contaminated with vesicular secretions from an infected person. Chickenpox is highly contagious. Most people become ill in childhood, and because the disease leaves a lasting immunity, adults are immune to the disease and recurrence of chickenpox is rare.
Vaccine: Varillix
It is worth vaccinating your child against meningococci, which cause meningitis. Due to the large number of vaccinations, we recommend immunizing after the first year of life. Single-dose vaccine Neisvac C.
In the fall, it is worth vaccinating children against influenza. Especially those who go to kindergartens and nurseries. Vaccinations are also recommended for the elderly, those with chronic diseases.
Children and young people going to summer camps and colonies are advised to get vaccinated against tick-borne meningitis, the so-called tick vaccine. The vaccine is a three-dose vaccine. The second dose is administered between one and three months after the first dose, the third between six and twelve months after the first dose.
NOTE: vaccination does not protect against Lyme disease.

The community and family nurse provides comprehensive care to the patient, their family and the environment. Her tasks within primary care include prevention and education, as well as performing diagnostic, therapeutic or rehabilitation procedures (based on guidelines obtained from doctors) and care services. MIDWIVES CLINIC
Parents or guardians who wish to provide care to their child at our clinic should submit a declaration to the selected NZOZ "Twój Lekarz" unit (the form is available in the "For the patient/Become a patient" tab). When filling in the declaration, it is necessary to have the data of the person who will register the child for insurance (name, surname, PESEL). When registering a newborn, it is also necessary to provide an immunization card (so-called vaccination card) issued by the hospital where the child was born. After completing the formalities, your child will be provided with care:
