Tax Identification Number (NIP) is mandatory when issuing sick leave certificates.

We would like to remind you that in connection with the regulation of the Minister of Labor and Social Policy of 01.10.2010, sick leave forms have changed. The condition for issuing a sick leave is the mandatory provision of the PESEL number and the patient's personal NIP number or passport number in the case of patients who have not been assigned these numbers.
