Registration
Registration with doctors takes place from Monday to Friday from 8:00 a.m. to 6:00 p.m. By phone at 71 311 12 36 or online. Home visits are carried out from Monday to Friday in the afternoon or other hours after prior arrangement. Visits are accepted until 11:00 at the clinic's registration or by phone.

Procedure for registration of health care services financed from public funds:
- Registration takes place directly or through a third party on business days:
- at the clinic’s registration desk during its opening hours,

- Online registration requires a patient account. In case of technical problems, the IT department provides assistance (e-mail address: internet@twojlekarz.net). Appointment dates available in electronic registration are constantly updated according to the availability of doctors.
- Registration for prescription visits (extension of chronically used drugs) is done via: e-mail (recipes@twojlekarz.net), contact form, text message (690 010 399), telephone.
- The services are provided as soon as possible, taking into account the dates available in all NZOZ Twój Lekarz facilities.
- In cases justified by the patient’s health condition, services are provided on the day of registration; if there are no places available, a medical triage is carried out (a sudden illness card is completed with the medical staff), on the basis of which a decision is made regarding further proceedings.
- In cases that do not require a visit on the day of notification, it is possible registration for the following days.
- If a doctor’s visit is scheduled several days in advance, the patient receives an SMS reminder 48 hours before the visit. Please confirm your reservation by replying YES or NO if you wish to cancel your visit.
- A patient can only have one active appointment reservation with a primary care physician. The above does not apply to visits to the Well-Child Clinic.
- Planned visits to the Well-Child Clinic - vaccinations and patronage - are scheduled by the medical staff (we inform the legal representative about the date of the visit by phone, and if necessary by registered mail). Balances are scheduled by the parent or guardian at the selected Well-Child Clinic.

- Changing the date of a scheduled visit. In the event of circumstances that could not be foreseen at the time of scheduling the visit (sudden absence of the doctor, inability to provide services due to unforeseen circumstances) that make it impossible to carry out the visit, the patient will be informed as soon as possible by text message or email about the need to contact in order to schedule a new date. In the event of an unforeseen event on the patient's side, please inform the clinic immediately about the need to cancel or change the date of the visit. Canceling an appointment allows other patients to receive advice at the freed-up time and helps reduce waiting times for appointments.

For organizational reasons, in order to shorten the waiting time for a connection as much as possible, please contact us after 10:00.
- During registration, the following are required: the patient's PESEL and e-referral PIN (does not apply to gynecological and psychiatric clinics).
- In the event of a subsequent visit to the same clinic within 2 years, a referral is not required.
- If a doctor's visit is scheduled several days in advance, the patient receives a text message reminder 72 hours before the visit. Please confirm your reservation by writing YES or writing NO if you cancel your visit.
